What is the cause and prevention of infertility?

Now a days infertility is the one of the leading problem in all over the world especially India.I would like to know the cause and preventive measures of this because detection will give good results.

Hi, Infertility can be dealt under two topics. One male infertility and two female infertility. In either case the female partner is unable to conceive after two years of sexual life (if i am correct). Lots of reasons are there for both partners to be infertile. One more important cause is it may not be on either partners but the compatibility between them.

For male partners it can be kept short. It may be due to less sperm count <20 millions/ml, the motility of sperms less than 30%, and the morphology of sperms.
Smoking contributes to most of the male infertilities apart from chromosomal abnormalities.

For female partners it can broadly looked as chromosomal abnormalities like turner females, irregular menstrual periods and other hormonal changes resulting in failure of release of ovum. Even if the ovum is released it may not successfully fertilize, even if it fertilizes it may not get implanted due to the fallopian tube defects. So the reasons are wide ranging from anatomical defects with tubes, uterus, to hormonal changes due to hypothyroidism, PCOD (polycystic ovarian disease), obesity (ultimately disturbing hormones).

One important thing I would want you to know is the male or female might be infertile to a specific partner, this happens because the female develops antibodies to the sperms of that particular male as it recognizes it as an antigen. So in case of less sperm count what kind of measure can be taken and what should tht person do?

Is there no hope for that couple?

Hi, There is hope and it depends. Again here there are plethora of causes for decrease sperm count. we should investigate the cause and then target the defect. If the cause is a varicocele, then surgical correction can o miracles. If the cause is obesity, it should be checked. If it is due to exposure of testicles to heat, or tight underwear then it should be addressed. If it is an infection then antibiotics will do.

There are drugs like Clomiphene citrate, HCG, HMG , bromocryptine (to some extent) that can cause increase in sperm count. The ultimate solution if the count is not increasing is to go for an IVF (in vitro fertilization) with counts as low as <5 million/ml.

There are few drugs that can cause this like Cimetidine, spironolactone, etc. One should even avoid smoke, alcohol, cocaine etc

It is said in sometimes that lack of calmness mind during bed time (in both husband and wife) could cause infertility and late in conceiving. It is also advised not to quarrel or discuss about the family matters during bed time by the couples for better effect.

Hi Ramachandran, I cannot comment on that. But it may be true.

@simmi, hormones are influenced by a lot of factors. One important measure you can take is to maintain ideal BMI as obesity causes hormonal changes due to increased production of Oestrogens from fat and diseases like PCOD, metabolic syndrome, insulin resistance diabetes.

Yes we can make the hormones regular by administering HRT hormone replacement therapy and closely monitoring.

@Anonymous, in case of less sperm count in males, it can be corrected / increased to a great extent through the practice of certain yogasanas and certain vegetarian foods that help in increasing the sperm count in males. However, if it is too less, then I think no hope other than adoption of a child / children. By this way the couple will be happy to have a child though not of their own blood and also they would be giving a life to an orphan child.

The reasons for male and female sterility are many some of which are curable while others are not, in case it is inherited. In males, it is mainly the low sperm count or even premature ejaculation which might result in infertility. However, a healthy and nutritious food, tension-free life, healthy life style, regular exercises, in particular certain yogasanas which one need to do under the guidance of a yoga expert are helpful in making a male fertile to a great extent.

And regarding female infertility the reasons are many like family history of infertility, hormonal imbalance, Poly cystic Ovary Syndrome (PCOS), poor ovulation due to poor dietary habits, malnutrition right from childhood, anemia / bloodlessness, severe endometriosis, egg health, fear of the conceiving or the act as well and various other reasons lead to infertility in women. And most of these can be treated to a great extent.

However, the couple need to plan well in advance if they really want to have children and they need to act accordingly. First of all, they should come out of certain inhibitions and they should discuss freely on this issue and should feel free as well while they meet instead of getting tensed up. They should know the ripe time of the woman which is usually from the 11th day to 19th after menstrual cycle as this is the ovulation period when an egg is released from the ovary. Here most of the women need to keep in mind that egg health / quality of egg is very important for conceiving, in case a poor quality egg is released then either it fails to get fertilized or even if it gets fertilized, it fails to implant in the womb, thus leading either miscarriage or abortion. So egg health is also very important for conceiving which can be cured by the intake the highly nutritious food several months before conceiving. Similarly regular exercise like cycling or belly exercise help in the proper functioning of the ovaries and thus help in regular ovulation as well. However, the both the husband and wife need to aware of certain facts on the proper functioning of reproductive organs and there need to be a proper planning as well for conception by the women well in advance

Nice post by kalyani. There are few things to correct.
Even in low sperm count there is a chance of conceiving by intrauterine insemination or IVF(invitrio fertilization) otherwise called test-tube baby. The last option is to adopt.

Fear of conception will not cause infertility. If you mean fear conception leading to avoidance of intercourse, then it is not infertility as infertility is failure to conceive after intercourse.

The ripe time is 11 to 19th day after menstruation(start of bleeding) not menstrual cycle and this is very important and priliminary knowledge to prevent infertility.

Regarding effect of exercise on ovulation, there is no certainty and proof to my knowledge. Let me tell you one thing. Exercise definitely keeps us healthy in all aspects but strenous exercise will cause infertility as in athletes. It has a scientific reason. Influence of diet on ovulation has also not been under study.

Thank you Nikhil Ratna for the compliment. Yes, fear of the act particularly in females makes it difficulty for the female to conceive.

And hip exercise in particular helps in the rich supply of blood to the ovary thereby making the ovaries to function properly.

Hi Kalyani, yes pelvic exercise helps in conception and delivery. Regarding delivery is well known as it relaxes the pubic ligaments and pubic sypmhysis etc but regarding conception is not well known.

I agree regarding fear of act as it results in frigidity of muscles and make the act difficult and attainment of orgasm is not possible. During orgasm the pelvic muscles contract after the deposition of sperms and mechanically aid the propulsion of semen and result in early and healthy fertilization.

You are welcome Remya. I am not sure about the foods that increase sperm count but I read that Soya and fatty (saturated)food can cause a decrease in the sperm count. Smoking causes decreased sperm count. So I think removing these factors would be sufficient to have fertility as it is a natural process. I can say that diets rich in vitamins and anti-oxidants can increase the count and quality of sperms.

Again PCOD is seen more in obese women. It means it is not an isolated disease and it is a part of whole lot of obesity complex. A less calorie diet helps to fight this problem.
