There are two ways of applying for passport:
1. online form submission
2. submit form at Passport Seva Kendra
Procedure to submit form online:
step 1:Go to your browser and open the below link
http://passportindia.gov.in/AppOnlineProject/user/RegistrationBaseAction
step 2:Log with your Passport Seva user account.
step 3:Choose the Apply Passport Online menu
step 4:Fill the given form with required details and press submit button.
step 4:Now you have to must be present on Passport Seva Kendra (PSK) with your original documents to complete the procedure.
Another way to get passport, submit form at Passport Seva Kendra with your required original documents.

Mr. Vivek has provided a good information I want to add some more information. For registration in the site you have to create a password which is valid only for 90 days after that you have to generate new password.
When you are filling form provide all the information using your documents and it also needs two referrals and their mobile numbers.
The documents you need are voter ID, Bank pass book and birth certificate. By using only three documents you would be able to get passport easily.
You have to manage an appointment so after filling form take an appointment and attend at pass port seva kendra on time with all the documents. Do not forget to take a print of your form. You have to submit Rs. 1000 for normal and Rs. 2500 for tatkal passport. You can also select pre or post police inquiry.

Go through this article to know about the procedure for applying for an Indian passport. You just need to go the nearest regional passport office, obtain a blank application after paying the specified amount. Fill in all the required columns and then submit the same along with all the required documents. Generally it takes two - three weeks to get the passport including police verification. However, in case you want to obtain the passport quickly, you need to apply for the same under Tatkal scheme for which you need to pay some extra amount and this will take 5-7 days to obtain a new passport. As per the recent announcement by the government, it takes 3-4 days to get a passport under normal scheme, but not sure how far this is effective in our country
