Here is a sample letter that you can send to banks, government departments etc to request a change of address in their records. Remember to include a copy of a valid address proof that shows your name and new address. Change of Address request without a valid proof of address may be rejected.
When you use the following format, remember to change the date, your name, bank name etc. Also, correctly mention the branch name of the bank.

Address for communication
Most banks will accept the change of address request at the local branches. However, in case of new generation banks like Citibank etc, you may send the change of address request to their corporate office.
Usually, the following documents are accepted as valid address proof:
1. Drivers license
3. Aadhar card
4. Election id card
Some banks require the address proof to be self attested. So, to be on the safer side, sign on the copy of the address proof before you hand over it to the bank.
Once you submit the documents and letter, it may take up to 30 days for the change of address to take effect. So, be prepared to receive the mails in the old address for few more weeks.

|Usually to change information like contact number, address or any other such information, the banks have a "Data Change Form". This form could be used to change many types of data. For example, I had two accounts in separate branches of a bank, with both of these I had associated a single Debit Card. When I used this debit card at the ATM of my own bank, I had the option to choose the account from which I wanted to withdraw my money. But when I used this card at an ATM of any other bank, I did not get this option and the money withdrawn was only from the default account or the account I had opened first. Whereas, I usually had balance in the second account due to my relocation from one city to another. And if I didn't have balance in this default account, I faced great trouble in locating an ATM of my own Bank. So, I contacted the Bank and they informed me to fill in the "Data Change Form" on which I could request to change my default account. I did so, and now I have no problems withdrawing money.|
