Date: Write the date in dd/mm/yyyy format (i.e date/month/year)

XYZ Bank (Here write the name of the bank that has issued you the credit card)
abc branch (Write the name of the branch. E.g Somajiguda branch)
City (Write the name of the city).

Dear Sir / Madam

Sub: Request to waive off bank charges - Reg.

I hold an SB account with your bank for the past _________ years (mention since how long you have an account for e.g for the past 15 years) and my SB Account No. is xxxxx. (Write your Savings Bank A/c no. in place of 'xxxxx').

I have taken home loan from your bank and I have been promptly paying the installments for my loan. Unfortunately, for the first time there is delay in the payment due to health grounds and I have been charged by the bank for the delay.

Request you to look into my payment history, consider my case on health grounds and waive off the charges. Hoping to get a positive action from you in waiving off the charges.
