Dear Sir/ Madam,
I (name of the customer) hold the Savings Bank Account No. (indicate the Savings Bank Account No.) in your Branch. I would further like to state that after the update of my Passbook, I have found that the Bank has deducted Rs. (indicate the exact amount) on (indicate the date of deduction). Unfortunately, in the Passbook, a code has been indicated as remark which is not understood by me.

In view of above, you are requested to give the details of the deduction as mentioned above at the earliest. I am enclosing the photocopy of the relevant page of the Passbook.
