Requesting customer to fill the Customer Satisfaction (CSAT) form

In most IT companies, it is a trend these days to ask the customer for their candid feedback so that the quality of services be improved accordingly. The customer is sent an e-mail with the form attached and he is requested to take out some time to fill it and share it back. It is an annual exercise mostly but depends from project-to-project and client-to-client.

The data helps the organizations to evaluate themselves. They also come to know about the areas of improvement.


Sample Usage

Dear Tom/Jerry,

I belong to the Program Management Office (PMO) at ABC (name of the organization). As a constant endeavor towards continuous improvement, we conduct a customer satisfaction survey from time to time, to solicit customers' feedback and use their inputs to further improve our quality of services.

Request you to kindly share your candid feedback regarding the services provided by us in the attached CSAT form. Please pick the option you find most appropriate from the drop down provided against each category. The aggregate of these values is automatically computed to determine the CSAT score.

Please email the completed form back to me alone, to maintain integrity of this exercise.
