To 'call a spade a spade' means to tell the truth or to narrate things as it is. Spade is the name of one of the four suits in a pack of cards. Since it is famously noted that people mostly try to bluff while playing cards, an honest card player is very rare to find! Similarly, an honest and truthful man, who calls things what they are, is very difficult to find. In fact few people have the guts to call a spade a spade, that is to say, do not be afraid to come out with the truth.
