The steps in applying for student visa are as follows:

Directly contact to the institution

As per the current rule, you need to apply for a student pass from your chosen institute and not via the Immigration Department. Follow all the rules and regulations indicated by the institution. Then finally, the application will be submitted by the educational institution of permit Division of Malaysia.
Note that your passport must be valid for more than 1 year. If it is less than 1 year, your pass might be rejected.

Wait for the acceptance confirmation

Aspiring students will be informed of their application status by the Immigration Department in Malaysia through the institution that one has chosen. You might be accepted or not, the notification is usually sent within a week.

Send the required documents

You have to mail them the visa documents required or you can send it to the place they specify though transaction are made directly from the Institution. From that point, hte process will taken care of by the Educational Institute itself.

They will send a confirmation letter of your student pass

An approval letter for your student pass will be released by the Malaysian Immigration Department and be delivered to the Institution as per your acceptance. Then it will send this approval letter to you.

Inform the institution

You must inform the institution about your airport location, arrival date and time, flight number etc. before you go to Malaysia. A representative from your institute will greet you at the immigration check point at the airport. Then the visa will be issued to you on your valid national passport.

Wait for pass sticker

Within few weeks, your passport will be submitted to the immigration Department and a student pass sticker wil be attached to it.

The fees depends on the country but not more than US $29.41. The passes actual cost around US $17.65
Finally, once you have done all the proposed steps, you are now officially enrolled in and ready to be a part of your chosen Malaysian institution.
