It is not necessary that such a house helper is a full-time maid or a male servant only. The form is very simple and can be filled easily by the employer. You just need to fill the personal details of your servant and his/ her specialized area of help. Servant's fingerprint impression of each finger and thumb are also required. You have to fill Part I, Part II-A and Part II-B only. Part III of the form is for police use only and to be filled by authorized person only.
